For current clients
Certificates of insurance, new trucks and drivers, policy changes and claims. Send the request here and a licensed agent on your account picks it up.
The request we get most from trucking companies and security guard firms. Give us the certificate holder’s full legal name and address, plus any additional insured, waiver of subrogation or primary and noncontributory wording the contract asks for.
For a vehicle, send the year, make, VIN, garaging address and the date you need coverage. For a driver, send the full name, date of birth and license number and state. Most carriers run a motor vehicle report before approving a new driver.
New address, new location, a change in payroll or revenue, a lienholder or mortgagee update, or removing a vehicle you sold. Tell us the effective date you need.
Report the claim to your carrier’s claims line first, which is printed on your declarations page and ID cards. Then let us know, and we’ll help you follow up with the adjuster and make sure the carrier has what it needs.
Copies of your declarations page, auto ID cards, loss runs for a renewal or new quote, and proof of coverage for a lender or landlord.
One request per form works best. If a contract or letter spells out the certificate wording, email it to info@treeinsuranceagency.com with your business name in the subject line.

A licensed agent will confirm your request within one business day. Changes to coverage aren't in effect until we confirm them with you in writing.
If a job, a load or a site visit is waiting on a certificate, call (888) 994-7692 after you send the form and tell us the deadline.
To report a new claim, call your insurance company’s claims line first. It’s printed on your ID card and declarations page, and most carriers take claims around the clock.

The short answer
Fill out the service form on this page, choose Certificate of insurance, and enter the certificate holder’s legal name, mailing address and any wording the contract requires. Email the contract or the holder’s request letter if it lists additional insured or waiver requirements. Most certificates go out the same business day when the policy already meets the requirements.

A certificate of insurance (COI) is a one-page summary that proves you carry coverage. It lists your policies, limits and dates, and names the person or company who asked for it, called the certificate holder. Brokers, shippers, property managers and general contractors ask for one before they’ll put you to work.
For trucking, a broker or shipper usually wants to see auto liability and cargo limits. For security guard companies, property managers and event clients usually want general liability, and often additional insured status. A certificate only reports the coverage you have. If a contract requires additional insured status or a waiver of subrogation, the policy itself has to include it, and some carriers need to add an endorsement first. Send the contract early so a missing endorsement doesn’t hold up the job.

Most certificates go out the same business day when your policy already has the coverage and limits the holder asks for. If the contract requires an endorsement, such as additional insured status, the carrier may need a few days to issue it first. Tell us your deadline in the request so we can prioritize it.
An additional insured is a person or company your policy also protects for claims arising from your work for them. General contractors, property managers and shippers often require it. Listing someone on the certificate alone doesn’t make them an additional insured. The policy needs an endorsement that grants it, and we request that from your carrier.
Send the vehicle’s year, make and VIN, or the driver’s name, date of birth and license number, through the form on this page. The carrier reviews the driver’s motor vehicle record and confirms the change. Coverage for the new truck or driver starts only once we confirm the change with you in writing.
Call your insurance company’s claims line first. It’s printed on your ID card and declarations page and is usually open 24 hours a day. Then contact us. We’ll help you follow up with the adjuster, track down documents the carrier asks for, and answer questions about what your policy covers.
